PROGRAM PARTICIPATION AGREEMENT
Program Overview
Plumitif Empowerment Academy offers psychoeducation-based wellness coaching programs to help individuals learn and practice skills for stress management, communication, emotional regulation, self-esteem improvement, and overall quality of life. These sessions are conducted online via platforms such as Zoom, fostering a supportive and interactive group dynamic. Participants are encouraged to turn on their cameras and actively contribute to discussions. Recording is not permitted.
This program is not therapy, nor does it provide a diagnosis. It is a wellness coaching program that incorporates psychological methodologies such as DBT, CBT, ACT, and mindfulness, among others, to equip individuals with practical coping skills and strategies for personal growth.
Each program includes access to an online forum for continued engagement, homework assignments, and additional resources. Graduates of the program will retain access to supplementary materials at no extra cost. The curriculum of any, or all our programs, may be adjusted at the discretion of the coach based on the needs of the group.
All potential clients are invited to a complimentary 30-minute consultation before enrollment to address questions and discuss program suitability.
For minors, a parent or guardian must provide consent before enrollment.
Participation in our programs, activities, and the purchase of our products and services is entirely voluntary. You can choose to participate fully, partially, or not at all. I understand that I can withdraw at any stage.

By signing below, you acknowledge and agree to the following:
- Age Requirement
- If you are under 18 years old, a parent or guardian must provide consent for your participation.
- Program Nature
- I understand that this is not therapy, nor does it provide diagnoses or medical treatment.
- Program Flexibility
- I acknowledge that curriculum topics and scheduling may be adjusted based on group needs and at the coach’s discretion.
- Commitment & Participation
- I understand that active participation and commitment are required to achieve positive outcomes.
- Confidentiality & Privacy
- Plumitif Empowerment Academy is committed to maintaining the confidentiality and privacy of all participants. Information shared in sessions will remain confidential, except in cases where there is a risk to the safety and well-being of a participant.
- Online Forum & Resources
- I understand that I will have access to an online forum for discussions, homework assignments, and additional learning materials.
- Privacy Policy & Group Rules
- I confirm that I have read and understood the privacy policy, consent form, and group participation guidelines.
- Parent/Guardian Consent (if applicable)
- If I am enrolling a minor, I confirm that I am the parent or legal guardian granting permission for their participation in the program.
I acknowledge that I have read and understood the Plumitif Empowerment Academy Consent Form & Program Participation Agreement. I voluntarily agree to participate (or allow my minor to participate) in this program.
I understand that all personal information shared during this program will remain confidential unless disclosure is required due to a safety concern (e.g., risk of harm to self or others).
